45.  Choice B is the correct answer.  When the intrinsic rate is over 150, it needs to be determined if it a narrow complex tachycardia.  If so, we can say that the pacemaker is coming from outside of the ventricles so we refer to it as supraventricular tachycardia (Paroxysmal atrial and junctional tachycardia).  A-Fib with RVR is an irregular rhythm and has a rate over 100.  Ventricular tachycardia is a wide complex tachycardia.
